Introduction to Activator 2 – Functional Orthodontic Appliance for Class II Correction

Activator 2 is an advanced variation of the classical Andresen-Häupl Activator, designed specifically for the treatment of Class II Division 1 malocclusion characterized by a retruded mandible and deep bite.

The appliance provides sagittal control by using a guiding channel and stabilization wires, which reposition the mandible anteriorly, stimulate mandibular growth, and optimize the occlusal relationship through non-invasive orthopedic principles.


Structural Components

  • Bimaxillary acrylic body: custom-fitted to the occlusal surfaces of the upper and lower posterior teeth.

  • Guiding channel: a mechanical groove that stabilizes the mandible in a forward position during appliance wear.

  • Guide wire: ensures sagittal control and limits free mandibular movement compared to the classical Activator.

  • Labial bow and anterior springs (optional): provide torque control for upper anterior teeth when needed.


Clinical Indications

Activator 2 is indicated for:

  • Skeletal Class II Division 1 malocclusion due to mandibular deficiency.

  • Patients with deep overbite and reduced mandibular angle.

  • Individuals in the growth spurt phase (ideally 9 to 12 years old).

  • Functional therapy cases without major TMJ pathology or craniofacial syndromes.


Mechanism of Action

The therapeutic effect of Activator 2 is based on controlled mandibular posturing and stimulation of condylar growth. When the appliance is in place:

  • The mandible is held in an anterior-forward position.

  • Functional orthopedic forces stimulate condylar cartilage remodeling and increase mandibular length.

  • Upper incisors may retrocline under appliance pressure, while lower incisors are uprighted.

  • Posterior vertical eruption is limited, aiding in deep bite correction.
